The Grace Doherty Library and Centre College request the rights listed below to ensure we are able to use and preserve the histories collected through the Centre College Ghost History project for posterity and research use.

I hereby authorize Grace Doherty Library and Centre College and those acting pursuant to its authority to:  

• Use my contribution for the Centre College Ghost History project.

• Use my name in connection with these recordings.If you would like to remain anonymous, please let us know.

• Use, reproduce, exhibit or distribute in perpetuity and in any medium (e.g. print publications, video, internet/world wide web, and/or other media formats and platforms and including future technologies not in use at present) these materials for educational and preservation purposes.

I release Grace Doherty Library and Centre College and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of Grace Doherty Library and Centre College in perpetuity.
